Patient Centered Medical Home

The medical home model holds promise as a way to improve health care in America by transforming how primary care is organized and delivered. Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.

"Patient-centered primary care is defined by the Agency for Healthcare Research and Quality as care that is relationship-based with an orientation toward the whole person, and that includes partnering with patients and their families to understand and respect each patient's unique needs, culture, values, and preferences. Our Physicians

All of our physicians are licensed by the Florida Board of Medicine and are board certified by the American Board of Pediatrics, which means that they have and maintain medical knowledge and experience about diseases, symptoms, and developmental standards specific to children from birth to the age of 18 years. They are each also Fellows of the American Academy of Pediatrics and committed to knowing, continually learning, and keeping informed about current medical research in pediatrics, and adhering to recommendations and professional standards for best practices in pediatrics. Our physicians’ commitment to maintaining the highest professional and ethical standards means that they use their many years of clinical experience and their knowledge about current medical research and standards to make decisions about the care of individual pediatric patients and to continually improve the quality of the services rendered to our pediatric patients and their families with their personal attention and compassion.
